02. Chamber Enlargement and Vectorial Hypertrophy Criteria

Adult chamber hypertrophy reflects chronic pressure overload (concentric hypertrophy) or volume overload (eccentric dilatation) resulting from systemic hypertension, valvular heart disease, or cardiomyopathies.

3. Right Ventricular Hypertrophy (RVH)

  • Myers & Sokolow Criteria:
    1. $R/S$ ratio in lead $V_1 > 1.0$.
    2. $R$ wave in $V_1 \ge 7\text{ mm}$.
    3. $S$ wave in $V_5$ or $V_6 \ge 7\text{ mm}$.
    4. Right Axis Deviation ($>+90^\circ$).
    5. Right Ventricular Strain Pattern (ST depression and T-wave inversion in $V_1 - V_3$).

4. Multimodality Diagnostic Investigations

  • Transthoracic Echocardiography (TTE): Calculates Left Ventricular Mass Index (LVMI: LVH defined as $>115\text{ g/m}^2$ in men, $>95\text{ g/m}^2$ in women) and Relative Wall Thickness (RWT: $>0.42$ defines concentric hypertrophy).
  • Cardiac MRI: Most accurate standard for volumetric myocardial mass quantification.

5. Evidence-Based Clinical Management Protocols

  • Intensive Antihypertensive Therapy: Angiotensin Receptor Blockers (ARBs) and ACE inhibitors promote the greatest degree of LVH electrocardiographic and anatomical regression.
  • Anticoagulation in Bayés Syndrome: Prophylactic oral anticoagulation (DOACs) considered in advanced interatrial block with high CHA2DS2-VASc scores to prevent embolic stroke.
